Private ADHD Titration

Royal_College_of_Psychiatrists_logo.pngThere is currently a national shortage of medication to treat ADHD symptoms. This is due both to manufacturing issues and an increase in demand worldwide.

Our service is able to offer private ADHD titration as an alternative to waiting for NHS funding through Right to Choose. Our assessment fees include the cost of a single prescription.

Titration

A highly experienced psychiatrist consultant will conduct a thorough evaluation of your ADHD. This includes medical history and an assessment using the common ADHD rating scales and questionnaires.

After your evaluation, you will be informed that medication may be beneficial for your particular situation. This will be discussed at your follow up appointment (which can be done in person, via video link or via phone). Your clinician will explain the various options for medication and, if you choose to start on medication we will suggest titrating the dose to maximize the balance of benefits and side effects for you.

This process could take several weeks, and it involves gradually altering your child’s medication until the proper balance has been achieved. It's not uncommon to to titrate as high as three different medications until you discover the one that best fits your child's ADHD. It's important for parents to take part in this process of titration and keep a close eye on their child's behavior so that they can inform the doctor if the medicine is helping or causing side effects.

Unless you have decided to return to NHS care the medication you take will be provided with a private prescription which you'll need to pay for at the pharmacy (this is not included in your consultation fees). You must attend six monthly appointments to monitor your medication - this includes blood pressure and pulse readings. Annual reviews are also mandatory.

Follow-up appointments

Once the evaluation has been completed, you'll be able to schedule follow up appointments with the psychiatrist as appropriate. The appointment is usually done through a zoom video call. However, in some instances, it could be possible to have an audio-only appointment to be scheduled (again with an additional cost).

At this point we will discuss any treatment options we have and come up with a unique ADHD medication titration plan. This will include a review and discussion of your medication dose, as well as any adverse effects you might be experiencing. The cost of these titration consultations is PS150 for face to face sessions and PS60 for telephone or online video consultations. Medication costs are additional and are paid directly to the pharmacy.

The psychiatrist will ask questions about your family history, the effects of your adhd titration private symptoms on your life, and any relationship problems or mental health issues that you might be suffering from. This is due to the fact that ADHD is a genetic condition and it is essential to examine any family history that could indicate that you're at risk of developing the disorder.

It is important that you are honest and open with your doctor during this evaluation, as the information they gather will be used to determine whether your ADHD fits the diagnostic criteria. This is a crucial step, as a wrong diagnosis could result in an inappropriate prescription of medication.

If you decide to be treated for ADHD we'll schedule an appointment to follow-up with a psychiatrist who specializes in this field. This can be done within the first few days of your diagnosis if you want to start treatment right away or within two weeks if prefer to take time to think about the implications of your diagnosis and the possibilities available to you.

We suggest that if are unsure about your preferences, talk with your GP prior to booking your private examination. Some providers have different guidelines regarding whether or not they require a GP letter. Make sure that your GP accepts the Shared Care Agreement, which means you will be referred to them once you leave the clinic for prescription medication.

Medication

For many people who seek a private diagnosis of ADHD medication is an important part of their journey. The process of getting prescriptions from an NHS psychiatrist can be a challenge and long-winded. NHS waiting lists can be extremely long, and GPs may be reluctant to refer their patients to specialist services.

One method to circumvent this is to ask your GP to refer you through Right to Choose (see psychiatry-uk and ADHD 360). This means that you can choose an organization that offers an assessment and titration. This can help you reduce time and be more convenient, particularly in the event of a hectic schedule.

Once you've been diagnosed, your doctor will assess you to determine if need medication titration. You'll have to complete forms that vary by doctor however the most commonly used ones are self-reporting forms and medical histories that go to your childhood and into adulthood. They will help your physician identify any factors that might mean you shouldn't take ADHD medication if prescribed.

If you are being titrated the medication will consist of a mixture of stimulants and non-stimulant drugs. These will be carefully assessed and monitored to determine the best combination for you. The titration procedure can be a lengthy process and may require several extra appointments before your dose is stable.

Depending on the kind of medication you are taking, follow-up appointments can be scheduled either monthly or quarterly. Your physician will make sure that you are stable with the medication, record any side effects that you've experienced and make any necessary adjustments to ensure that you remain on a dose that is safe. You will also be required to attend a regular health check which can include blood pressure readings, pulse and weight and an annual review.

For some people, it may be that they are content with their medication and don't require a further discussion about their titration. However, for those who require further discussions regarding their titration and treatment plan it's possible to be passed back to the same private clinic that made their initial assessment. In this case, the titration will be included in the consultation fee. The medication will be provided through an individual prescription. It could cost more than an NHS prescription because the pharmacy will charge for the medication.
